Adhesive strains lead to acute pyelonephritis (APN) without major VUR, inversely non virulent commensal strains are found in APN with major urological anomalies.We could predict high grade VUR accounting on bacteria belonging to group A and absence of pap GII with an acceptable specificity, but bad sensibility. Analysis of pap GII, pap C and group phylogenetic A and B1, all together with PCT serum levels may help to predict high grade reflux in children with UTI. As known, PCT has 100 sensibility and a bad specificity; it could therefore be complementary with virulence factor analysis.
