Hi Linda,

It is interesting study,

I think, it will be better to study serum NGAL in these cohort of patients. Serum NGAL is more sensitive than urin NGAL.

Thank you for your comment

You are probably right. I think that the combination of UTI /urosepsis and nephrotoxic antibiotic is important cause of AKI. We know that biomarker performence, especially NGAL , in sepsis is worse than in other clinical settings. In addition, the major molecular form of HNL/NGAL secreted by neutrophils is dimeric, whereas the major form secreted by HK-2 cells is monomeric. The development of molecular form-specific assays for HNL/NGAL may be a means to identify the origin of HNL/NGAL in urine and construct more specific tools for the diagnosis of AKI in setting of UTI.
